Insurance and Payment
In order to provide you exceptional care, our office does not participate In-Network with any insurance companies or discount plans. As a courtesy we can provide you the necessary documents for self-submission to your insurance. Patients are responsible for cost of treatment at the time of service. Forms of payment we accept include, CASH, Check, HSA/FSA, and Debit with no additional fees. Visa, Mastercard, and Discovered credit cards are accepted with a 3% surcharge.
